Costs and Insurance

Members pay an annual membership fee to be enrolled in the membership primary care program. Office visits will require a copay at the time of service that is part of your insurance contract.  Our office will bill your insurance and accept reimbursement for services as payment in full. We accept all types of insurance, including commercial insurance, Medicare and HMO plans.  If you have Medicaid, Care source, or cash pay we can provide membership with some exceptions.  Contact the office directly for additional information.

Do you take my Insurance and Medicare?

Insurance

Your membership ensures access to your provider who is Board Certified in Family Medicine.  Medical services will be billed to your insurance and we accept what is reimbursed. The membership fee will be used to pay for all your co-insurance, and deductible associated with services from this practice.

Non-insured patients:  Office visits will be covered.  We will work with you to find the most affordable options for immunizations that will need to be administered outside of this office. 

Medicare

We do participate in traditional Medicare as well as most Medicare Advantage programs (Medicare HMOs).

What is the cost of participation?

Membership in the practice is available for an annual fee. The fee works out to likely be less than your monthly cell phone and/or cable bill. With your membership fee, you have around-the-clock access to unhurried appointments, personalized care, and all the other features as previously stated. This means you receive superior personalized healthcare solutions when you need it.

Payment Options
Annually:  discount for memberships paid in full with one payment. 
Two Payments - Half at start and other half  2 months later.
Three Payments - first payment, then every 2 months until paid in full. 

Fourth Payments - first payment, then every 2 months until paid in full.
